Stiffness limits life in ordinary ways: reaching a top shelf, checking a blind spot, getting socks on, kneeling in the garden. Sometimes it comes from the joint itself, sometimes from muscle and connective tissue, sometimes from protective guarding after an injury, and sometimes from months of not moving through a full range.

Because the causes differ, the treatment differs. Stretching a joint that is mechanically blocked accomplishes little; mobilizing a joint whose surrounding muscles are guarding will not hold. The examination sorts out which situation you are in.

Where restriction usually shows up

Common patterns seen in outpatient practice include:

  • Shoulder motion loss after injury, surgery, or a long period of guarding
  • Neck rotation limits affecting driving
  • Hip stiffness affecting walking, stairs, and sitting tolerance
  • Knee extension or flexion loss after surgery or immobilization
  • Ankle stiffness limiting squatting, stairs, and gait
  • Thoracic spine stiffness contributing to shoulder and neck symptoms

How range is restored and kept

Treatment usually combines hands-on work to change the restriction with active exercise in the newly available range, so the nervous system learns to use and control it. Home work is short, frequent, and specific — a few minutes several times a day generally beats one long session.

Is stretching enough to fix stiffness?

Sometimes, but often not. If the restriction is at the joint surface or capsule, or if muscles are guarding protectively, stretching alone tends to give short-lived relief. Identifying the source of the restriction determines whether stretching is the right tool.

How long does it take to regain range of motion?

Recent restrictions often respond within a few weeks. Motion lost over months, or after surgery and immobilization, generally takes longer and depends heavily on consistent daily home work.
