How coverage, referrals, and billing work — and how to verify yours.
Coverage for outpatient physical therapy varies by plan. Deductibles, per-visit copays, coinsurance, visit limits, and prior authorization requirements differ even between plans from the same insurer.
Our participating plan list has not yet been confirmed for publication, so we do not list plans here rather than risk telling you something inaccurate. Call the clinic with your member ID and we will verify benefits and referral requirements before you are scheduled.
Referral rules vary by state and by health plan. A plan may require a physician referral even where state law does not. We check both.
Self-pay rates are available. Ask when you call and we will explain the evaluation and follow-up visit rates in advance so there are no surprises.
